The following forms are for those individuals who are currently serving as a deacon or congregations being served by a deacon. These forms are to be completed each year by August 31 and sent to Martha Wohlfeil in the president's office.

- Ministry Description FormThis form must be completed to obtain your provisional license - as we try to honor Res. 13-02A. A ministry description form, prepared by your supervising pastor, may accompany this form but will not be accepted as a replacement for it
